Bichectomy
By Dr. Victor Urzola

Bichectomy is a cosmetic surgical procedure in which the Bichat bags are removed in order to refine the lower third of the face, to achieve a facial contouring that helps to give a triangular appearance to the face, reducing the prominence of the lower part of the face and therefore, giving greater luminosity to the malar area.

Bichectomy

Bichat bags are fat accumulations found in the cheeks and their function is only in the breastfeeding period to keep the mother's nipple in a central position so that the baby can breastfeed. Bichectomy is one of the most requested cosmetic surgical procedures nowadays. The results are noticeable two months after the procedure is performed, once the inflammatory process is over.

It is a mistake to think that by removing this structure, with the passage of time you will notice more flaccidity in that area of the face, because the opposite is true, as there is less weight and volume, gravity will have less effect than expected without having removed the Bichat bags. As with any surgical procedure, a preoperative assessment by the surgeon and preoperative laboratory studies are required to determine if the patient is in perfect health to undergo cosmetic surgery.

It is usually performed under local anesthesia in a minor procedure room. Once the anesthetic is infiltrated, an incision is made inside the oral cavity in the cheek, so there is no visible scar.

Postoperative care is important, since as with any surgical procedure, the most frequent complications can be bleeding and infections. Therefore, in addition to oral antibiotics, it is recommended to consume liquid and soft foods during the first 48 hours of the postoperative period, as well as antiseptic and anti-inflammatory mouthwash to reduce the risk of these complications.

Bichectomy is a surgical aesthetic procedure that is performed in a short time, in a relatively simple way and with extraordinary results to give the face a more delicate appearance, achieving the facial harmony that we are all looking for.

Before and After

Bichectomy postoperative recommendations

Once you leave the clinic; here's what you need to know:

How should I expect to feel?

  • Pain: will be relieved with analgesics.

  • Swelling in the cheeks that gradually diminishes, disappearing around the third week.

How do I care for my incision?

  • When the procedure is finished, you will leave with 2 gauze pads in your mouth on each side, which you should remove 30 minutes after the procedure.

  • Dr. Urzola makes a small incision to remove the bichat pockets, so no sutures are placed.

  • You should avoid consuming grains, such as rice, lentils, beans, chickpeas, etc. Do not consume dairy products either, you can resume them a week after surgery.

  • Place cold compresses on the cheeks to reduce swelling for 5 minutes, no more than 3 times a day.

  • Avoid drinking alcoholic beverages and smoking. As for smoking, if you do, you will have scarring problems and slow down your recovery.

  • Rest for at least the first 24 hours.

What medication will I receive after surgery?

  • Analgesics.

  • Anti-inflammatory drugs.

  • Antibiotics.

  • Mouthwash. Immediately inform Dr. Urzola if you develop any discomfort or intolerance to any of the prescribed medications, but do not suspend or change them without medical indication.

When should I call Dr. Urzola?

  • Increased swelling or bruising.

  • Severe or increased pain that is not relieved by medication.

  • Rash, nausea, headache, vomiting or constipation.

  • Fever, chills.

Remember that it is normal for the face to be swollen for a few weeks, so you will not be able to admire the results until a month later, when the face will have recovered its normal appearance. The bichectomy will slim the face, refine and stylize the facial features.
